Good Shepherd’s Bridge to Home program prepares families for the next steps of going home, providing a supportive rehabilitation environment that teaches families how to transition safely during what can be an overwhelming time.
Bridge to Home is ideal for a variety of complex medical conditions and/or medical equipment needs, such as feeding tubes, breathing tubes and breathing machines.
Bridge to Home also addresses patient and family needs resulting from new or life-changing events that have resulted in significant changes to functional status and medical needs.
Our program is designed to:
- Provide compassionate, expert support for families to think, plan, strategize and organize care to bring children home safely and comfortably
- Deliver 24/7 inpatient pediatric rehabilitation care while an individualized transition-to-home plan is designed for the home environment and families’ unique needs
- Train parents/caregivers on every aspect of care, including how to manage, troubleshoot and adjust any medical equipment
- Navigate follow-up appointments, insurance and more to get families back home in a timeline that best fits their needs
